1. Fill out our Residential Service Application. Download a copy Here.
If you are constructing a new home (or development), you will need to download our Construction Application Here.
2. You will need a copy of:
a. Your lease agreement or escrow closing document.
b. Driver's License
c. Applicants for service will be subject to an online soft credit screening. A deposit of $200.00 may be required to activate service.
3. Return the completed Residential Service Application along with the required documents to MID.
